As a Clinician in the Intensive Aftercare Prevention program, you will provide home-based clinical services to children and their families to help preserve or reunite them following Northeast Parent & Child Society's stated purpose and mission of protecting children, preserving families, and strengthen communities. The Intensive Aftercare Prevention Program (IAPP) is a home and community-based program assisting families who have children in foster care or children who are at risk of being placed in foster care. You will report to the Program Coordinator and you will work remotely with travel to client homes required.

Responsibilities:

Provide home and community-based assessment, planning and treatment with individuals, groups and families with those enrolled in our Family Preservation services (the level of intensity / number of direct services hours per family is dependent upon the services in which one is employed)
Complete necessary all records and client information following agency, county, and state policy and funding requirements
Provide liaison contact with collateral resources such as referring agencies, schools, family court, and other agency personnel serving on behalf of a child and family as it relates to their treatment
Responsible for the clinical decisions when assigned and paired to work as a team with a Bachelors Clinical Case Manager.
Ensure provisions of regular treatment plan meetings and services plan reviews.

Requirements and Education:

Requires a Masters degree in Social Work, Mental Health Counseling or related field
Minimum of one year of post-graduate experience.
License Preferred: LMSW, LCSW, LMHC, or LMFT
Must have a valid drivers license.

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.

Responsibilities:

Level I - Provides care management services to children with low and medium acuities. The Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in person with the member.
Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
- Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
- Work with children, youth, families, and service providers to create a comprehensive Plan of Care
- Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
- Convene and conduct Interdisciplinary team meetings to review the Plan of Care; identify progress, and continued needs, and confirm ongoing eligibility
- Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
- Use NY State and Health Home information systems to input required Health Home documentation
- Comply with Medicaid and program billing regulations
- Complete all required and recommended training

Requirements and Education:

Level 1 - Requires an Associate's degree in Social Work or a related field, with 2 to 5 years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
Level 2 - Requires a Bachelor of Arts or Science degree, and two years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.

Job Summary	The Administrative Coordinator reports to the Associate Dean of Undergraduate Education and provides administrative support to the Office of Undergraduate Education (75%) and Office of Institutional Research & Assessment (25%) to coordinate day-to-day administrative activities for these offices. The Coordinator provides support for office related functions such as portfolio wide purchasing, event planning, and support to members of the Office of Undergraduate Education (OUE) portfolio.
Minimum Qualifications	High School Diploma/GED Five (5) or more years of related office experience Relevant combinations of education, training and experience may be considered.
Preferred Qualifications	Associate's Degree Experience with Banner/OSCAR or similar systems.

Job Summary	The Administrative Coordinator provides administrative support for Associate Vice President Public Safety & Emergency Management and oversees the day to day operations for the Department of Public Safety. The Administrative Coordinator will serve as the primary point of contact for both internal and external constituencies, assists and represents the Associate Vice President in communicating with constituents, and handles multiple issues and concerns as they arise. The Coordinator also performs fiscal and budget related duties for the department of public safety.
